Healthcare Tech Medical Chart Abstractor

  • Excellent first shift hours, Monday through Friday! May have occassional weekend and evening hours!


The Healthcare IT Medical Chart Abstractor is responsible for accurately entering medical data into the clients electronic medical records system. In this role, the Healthcare IT Medical Chart Abstractor allows our client to provide consistent and efficient quality services to their patients.


1. Enters data accurately and efficiently.

  • Accurately abstracts health information from patients paper or electronic medical charts.
  • Accurately enters data into the clients preferred electronic medical records system.
  • Copies/pastes, and types text, accurately, editing for formatting and spelling.
  • Uses knowledge of medical terminology and standard procedures to identify deficiencies in the data sources and resolves discrepancies by using standard procedures or returning incomplete documents to the team leader for resolution.
  • Verifies data and restores accuracy of entered data by correcting, deleting or reentering data.
  • Meets or exceeds productivity requirements.
  • Effectively communicates verbally and in writing any data inaccuracies, client issues and process issues to leadership so that they can be addressed in a timely manner.

2. Adheres to regulatory standards.

  • Maintains patient privacy in accordance with the Health Insurance Portability and Accountability Act (HIPPA) and any Confidentiality Agreements.
  • Ensures that data is stored in accordance with the prescribed procedures.

3. Communicate progress to the Team and Project Lead.

  • Generate and submit reports daily and as needed.
  • Provide accurate reporting of performance metrics and findings to the Team and Project Lead.

4. Works effectively with coworkers and supervisors to accomplish goals.

  • Requests and provides assistance to coworkers as needed.
  • Informs supervisor when tasks need to be reprioritized and / or when assignments or schedules need to change, or when assignments cannot be completed.
  • Communicates respectfully to internal and external stakeholders.

5. Provides excellent customer service.

  • Represent US&S in a positive manner when interacting with clients, coworkers and all other internal and external stakeholders.
  • Maintains a positive and professional manner at all times, including when under stress.
  • Adheres to appropriate and dress code guidelines.

6. Adheres to all policies and procedures, including those prescribed by the client and those included in the US&S Employee Handbook and the US&S, Inc. Safety and Health Program Manual.

  • Must have proficient knowledge of medical terminology, preferably from a classroom setting.
  • Must have experience in the healthcare and / or healthcare information technology field.
  • Must be able to pass pre-employment 10 panel drug screen and ongoing drug and alcohol screening
  • Required to present proof of required immunizations per CDC guidelines by inoculation or titer (Measles, Mumps, Rubella (MMR) and Varicella (Chicken Pox); proof of immunity is documented by record of TWO MMR immunizations or a positive titer. Varicella is documented by record of TWO immunizations or positive titer, proof of influenza vaccine in last 12 months, Hepatitis B immunization or signed declination form, proof of two-step PPD (TB) skin test within the last year, history of positive PPD must have chest x-ray.)
  • Ability to qualify for client badging
  • Authorized to work in the United States per the E-verify database
